A 51 year old man has multiple papules, nodules and plaques on face, neck, chest and back. Ear lobe infiltration, clinical d/d: Jessner’s lymphocytic infiltrate; Sarcoidosis; Lepromatous leprosy; Lymphocytoma cutis.

Thank you for your comments. 

The final diagnosis is:  Histoplasmosis.

The differential diagnoses to consider are:
Leishmaniasis.
Cryptococcosis.
Leprosy.
Syphilis.
