A granulomatous condition with giant cells and possibly caseation. Lupus vulgaris, leprosy, leishmaniasis and granulomatous rosacea should be considered in differential diagnosis. Staining for acid fast bacilli, tissue culture and PCR needed for pinpointing the diagnosis.

I think there are acid-fast organisms. Lupus vulgaris.

Maybe a cutaneous manifestation of Wegener Granulomatosis should be ruled out.
