74-year-old female with erythematous papules and plaques with scale on bilateral upper and lower extremities. History of small cell lung cancer on immunotherapy. R/O MF.

Immunotherapy (Pembrolizumab induced) lichenoid drug eruption 

hyperkeratosis, parakeratosis, scattered apoptosis, basal layer vacoulation,  

near to saw toothed rete, upper dermal edema with eosinophils, dense lichenoid reaction pattern,

Diagnosis: Pembrolizumab-induced psoriasiform dermatitis.

The eosinophils indicate that this eruption is drug-induced.
