68-year-old female . Patches and plaques since 2005. First on the trunk. Later also on the limbs.

There is compact hyperkeratosis with marked upper epidermal pallor.  The dermis shows haemorrhage and a mixed lympho-histiocytic inflitrate.

I suspect mixed nutritional deficiency, perhaps with an element of scurvy to explain the bleeding.

To me the lymphoid infiltrate appears atypical, nuclei are large with irregular contour. Could this be a progression or transfromation in MF, which would go with long history of patches and plaques as well.

The lynphoid infiltrate is atypical - morfologically and phenotipically. Progrssion of MF.

Ah yes, the long history would not be in keeping with nutritional deficiency.
