Case 455 (P. McKee) M 32 frontal baldness 

Miniaturised vellus follicle.  Fibrous “stellae”.

Case 1947  15.11.17  (A. Bakshi)  F 29 hair loss top of scalp 

Transverse sections.  Main feature is miniaturisation of hair follicles (terminal:vellus 1:1), with ↑telogen count (20%).  Some perifollicular inflammation (esp. at level of isthmus) can be seen as non-specific finding in androgenetic alopecia.  Sebaceous glands preserved (which they wouldn’t be in a true scarring alopecia).  No significant inflammation around hair bulbs or in stellae, which argues against AA.

 

Case 2179  16.10.18  (U. Sundram)  F 40 diffuse hair loss (*teaching) 

Mostly horizontal sections showing variation in follicle size with miniaturisation. Some fibroplasia but no inflammation.
