Confluent and reticulated papillomatosis (CARP)

Operations Pathhub

Case 843  10.9. 13  (M. Hurt)  F  15 scaly reticulated plaque on abdomen

Invagination of hyperkeratotic stratum corneum into epidermis is a histological clue.  This biopsy also shows the fixation artefact of perinuclear clearing and ballooning of keratinocytes, similar to “birdseye” keratinocytes seen in plane warts.

Case 1542   23.5.16  (M. Hurt)  F 36 intermammary

I was initially thinking of PRP.  However, the epidermis shows blunt-topped papillomatosis.  The stratum corneum does not show the alternating para/orthokeratosis of PRP, but rather characteristic orthokeratotic invagination of the epidermis.  Rather psoriasiform acanthosis with some papillary oedema.

DD: acanthosis nigricans; epidermal naevus.

Case 2782  5.3.21 (R. Logan)  M 25 pigmented trunkal rash (*teaching)

Characteristic changes of blunt-topped papillomatosis, and invagination of epidermis by hyperkeratotic stratum corneum.

Case 4128  16.11.22 (H. Diwan) 16 macular rash on chest (*teaching)

Case 4192  14.2.23  (U. Sundram)  F 19 hyperpigmented patches and papules

PAS -ve.
