Invagination of hyperkeratotic stratum corneum into epidermis is a histological clue. This biopsy also shows the fixation artefact of perinuclear clearing and ballooning of keratinocytes, similar to “birdseye” keratinocytes seen in plane warts.
Case 1542 23.5.16 (M. Hurt) F 36 intermammary
I was initially thinking of PRP. However, the epidermis shows blunt-topped papillomatosis. The stratum corneum does not show the alternating para/orthokeratosis of PRP, but rather characteristic orthokeratotic invagination of the epidermis. Rather psoriasiform acanthosis with some papillary oedema.
DD: acanthosis nigricans; epidermal naevus.
