All dermal. Lots of plasma cells and an interstitial GA-like pattern.
Desmoplastic melanoma may be S100+ve, SOX +ve, but Melan A and HMB45 –ve.
Case 2552 17.4.20 (R. Carr) F 70 plaque forehead
Low power view dominated by Jessneroid or lupus-like nodular lymphocytic infiltrate, but dermis too cellular with spindle cells and mitotic activity.
Case 2557 24.4.20 F 75 forehead (R. Carr) dual pathology with CLL
DF-like storiform pattern of spindle cells in papillary and upper reticular dermis. Nuclei rather too numerous and plump for DF. Mitotic figures visible. Diagnosis missed by one or two. Philip McKee’s adage of thinking about desmoplastic malignant melanoma for any spindle cell tumour. Deeper nodular aggregations of lymphocytes represent to co-existent CLL with both T- and B-cell IH.
Case 2920 15.9.21 (U. Sundram) M 70 previously biopsied ear lesion
“Scar” appearance in dermis composed almost entirely of melanoma cells.
