Folliculo-sebaceous cystic hamartoma

Case 751   2.5.13  (H. Diwan)  M 34 cheek lesion 

Could only see 2 views.  A lot of debate as to what to call this lesion.  I went with the majority verdict.  A flat endophytic instead of papillary/polypoid lesion, with vertical, deep penetration of a keratin filled infundibulocystic space which communicates with the surface. Rare follicular and predominantly radial sebaceous lobules. Focally dense stroma, lacking adipose tissue. Unremarkable epidermis 

Kazakov in "Cutaneous Adnexal Tumors (2013)"   lumps late stage of trichofolliculoma, sebaceous differentiation in trichofolliculoma, sebaceous trichofolliculoma and folliculosebaceous cystic hamartoma into one and the same entity.

Central sebaceoma component.  Adipose element (not normally seen at this site).  Not cystic in this case.

Similar features to others.  Absence of adipose tissue is a feature apparently.

Similar morphology to 2338.  Debate as to whether to call this FSCH or sebaceous trichofolliculoma 

Case 2338  3.6.19 (L. Yu)  M 76 nose 

Central cystic cavity filled with keratinous and sebaceous debris.  Often not connected to surface. Sebaceous lobules attached. 

Case 2685 21.10.20 (H. Diwan) M 78 cheek nodule 

Most thought Fibrofolliculoma/Trichodiscoma.   

Case 3073  18.4.22 (M. Abdel-Halim) M 15 progressive papules face, neck, trunk and extremities 

Case 4078 7.9.22 (H. Diwan) F 58 nose lesion 

Mitt-like sebaceous gland, with biphasic proliferation of follicular epithelium and stroma 

Fibrofolliculoma/Trichodiscoma. 

Case 4476  18.3.24 (M. Abdel-Halim) M 35 follicular papules face and neck
