Case 686 31.1.13 (H. Diwan) F 29 black lesion arm. (Malignant version)
S100-ve; CD163 +ve. Granular cells, rather pleomorphic with one abnormal mitosis. Diagnosis debated with DD below.
DD: histiocytic sarcoma (CD163 +ve – not pleomorphic enough.
Granular cell variant of epithelioid DF.

The granular cells here are distributed singly and in columns between the collagen bundles and adjacent to nerves.

Pseudoepitheliomatous hyperplasia overlying granular cell tumour. Pustulo-ovoid body of Milian seen in Fig. 4 – large eosinophilic granule surrounded by halo.
