Foreign body reaction

Case 2522  6.3.20 (R. Carr) M 80 forearm injury 

Complicated by Chromoblastomycosis (from thorn injury).  Nice birefringence of vegetable matter; Medlar bodies (stacks of copper pennies) characteristic of chromoblastomycosis 

Case 685   29.1.13 (P. McKee)  F 21 indurated lesion thigh 

Looked like silicone reaction to me, but PMK not sure of what was injected 

Case 942  31.1.14 (R. Carr) M 72 thumb ? inclusion cyst 

Wood splinter (parallel xylem tubes) shown nicely on polarisation.  Fibrosis and plasma-cell rich infiltrate.  Branching filamentous bacteria on Gram – actinomyces, possibly Nocardia spp.  They may show a characteristic beading pattern (not seen here). 

Case 1342   14.8.15 (N. Tiffin/R. Carr)  M 30 “cyst” cheek (HIV +ve) 

Large empty space lined with eosinophilic FBGC’s reacting to keratin flakes (old ruptured cyst).  Also shows lipoatrophy with small, uniform adipocytes in response to anti-retroviral drug 

Case 1463   2.2.16  (H. Diwan) M 25  groin lesion 

Epidermally-lined cyst.  Surrounding “myxoid”, foreign-body reaction from steroid injection 3 months before.  Not all views visible. 

Case 1653 26.10.16  (H. Diwan)  M 70 finger (veteran) 

Shrapnel!  Calcium; brown banana-shaped structures; yellow-blue birefringence (similar to oxalosis) 

Case 2990  22.12.21  (H. Diwan)  M 10 2 year history of neck papule 

Foreign-body granulomatous reaction.  Thought to be tick mouthpart. 

Case 3030  16.2.22 (H. Diwan) F 55 forearm - ?cyst or infection 

Overlying eroded lichen simplex-like changes.  Injected steroid deposit in dermis represented by acellular, amorphous, somewhat vesicular material. 

Case 4011 6.6.22 (M. Abdel-Halim) F 28 hard nodules cheeks 

Granulomatous reaction to unknown filler, possibly silicone. 

Case 4031  4.7.22 (M. Abdel-Halim)  F 45 nodules, eyelids 

Granulomatous reaction to hyaluronic acid filler. 

Case 4118  2.11.22 (H. Diwan) 69 ?gender lesion, thigh 

Scar with injected steroid material.
